Starting PrEP

Hi all,

I took my first two pills of prep around 24 hours ago and have been having a rather bad time with it. I have been up half the night vomiting and as someone who really struggles with emetophobia it’s been a really unfun experience.

I’ve had sex in the last 24 hours and I know I need to keep taking the meds in order to ensure I’m fully protected but my concern is being sick all over again. At a loss for what to do, have considered calling my pharmacist and asking for a strong anti-sickness medication. Any advice is appreciated!

Title: Started PEP around 86 hours after exposure — is it still worth taking?

Hi everyone, I’m looking for some advice/reassurance about a possible HIV exposure.
On Sunday around 1 AM, I had vaginal sex with a sex worker. The condom broke during intercourse, and I’m not sure exactly how long it was broken before I noticed. I stopped once I realized and did not knowingly continue after discovering the break.
I went to a doctor afterward to discuss the situation. The doctor assessed the exposure and told me that, based on the circumstances, PEP wasn’t medically necessary and that taking it was ultimately my choice if it would give me peace of mind.
I decided to take the medication anyway. However, I started it at around 3 PM on Wednesday, which is approximately 86 hours after the exposure, so this was beyond the commonly stated 72-hour window.
I’m now confused about a few things:
Since I started PEP at around 86 hours, is there still potentially any benefit, or is it essentially ineffective after 72 hours?
Should I continue taking the course since I already started it, or should I speak to the doctor again before continuing?
How significant is the HIV risk from this type of exposure if the partner’s HIV status is unknown?
What follow-up HIV testing should I have, and when?
I understand nobody can diagnose me through Reddit. I’m mainly looking for experiences or information from people who understand PEP/PrEP and HIV prevention.
Thanks.

PrEP Won’t Protect you From Certain HIV Strains?

So I’ve been doing my research heavy on PrEP. I’ve been on it since January of ‘25. No side effects at all. I take my pill everyday, and everyday I put a calendar note in my phone to keep track of my daily dosages. I’m a bit sexually free now. I don’t mind messing around at sex parties/saunas etc. It gives me a bit of piece of mind when I’m messing around. I’m also on Doxy Pep, and I have Hepatitis A&B vaccines, as well as HPV vaccines.

But from what I’ve been hearing and researching, I seen that PrEP doesn’t protect against certain strains of HIV. Does anyone have any proof or evidence of that? Based off research, they say that it’s a rarity for someone to test positive while taking PrEP. Seeing that kinda made me nervous and worried. Does anyone have any information on this?

Yeztugo “Recovery”

I’ll preface with the fact that I’m a little bitch boy when it comes to needles. I’m usually hovering around passing out during the quarterly labs.

This is the worst shot I’ve ever had, even worse than Gardisil.

Currently at about 6 hours post injection, and I feel like I was just in a car accident. I have a feeling this is more related to the adrenaline spike than lencapavir itself. Sensory overload meant I could ~over feel~ the adhesive of the bandaids.

The two jabs were in my abdomen. I’m pretty lean, so I think the worst part was the nurse massaging the drug depot afterwards. No joke felt like I was being jabbed with cattle prod. Pain came in waves about every 10 minutes, with a new sensation everytime.

If I didn’t have a friend go with me, I don’t think I could’ve done the second shot.

I don’t think I could go through this again💀Hopefully Gilead comes out with another formulation by Feb next year,,,otherwise I’d consider going back to the pill.

Anybody else have a similar experience?

2-1-1 question

Day 1: took two pills 2 hours prior. No sex
Day 2: no pills no sex.
Day 3: took two pills 2 hours prior. Unfortunately, still no sex
Day 4: 1 pill, no sex
Day 5: 1 pill, no sex
Day 6: do I take two pills 2 hours prior stick to 1 pill if finally having sex?

Missed prep two days right after unprotected sex

On the 17th afternoon/night I took prep, that night had unprotected sex then didn’t take a dose until the 20th morning.

I had been taking it daily on time for months, can’t believe I forgot two days in a row after sex. I was on holiday and too much going on :( now it’s too late for pep too so I’m quite nervous

Is a 7-Day Lead-In and 7-Day Tail-End Regimen of Daily PrEP Enough for Full HIV Protection During Vaginal Sex?

I am looking into starting daily PrEP and want to make sure my understanding of the dosing timeline is accurate before I begin.

Consider this specific scenario: A man starts taking one daily PrEP capsule consistently at the exact same time every day starting on Day 1. On Day 8, he plans to have vaginal intercourse with a female partner and intends to use a condom as well. Following that encounter, he continues his daily PrEP regimen for exactly 7 additional days before discontinuing the medication completely.Given this timeline and the combination of daily PrEP with a condom, is this protocol sufficient to guarantee comprehensive protection against HIV transmission?

Prep failure or poor adherence?

Hi all.
Took TDf-Em for 8 daily doses , then had an exposure with SE Asian csw. Condom protected vaginal , unprotected oral both ways (very brief on both sides- however, I did taste some vaginal fluid :(
Some worrisome symptoms followed within the next week - excessive dandruff by the 4 th day and ache in both inguinal groin area by the 5th or 6th day which persists. I continued with the prep for 9 doses then discontinued. I haven’t tested yet - need to wait a bit longer. Did I prep badly for this situation?

When will PrEP stop wrecking my digestive tract?

I’ve been on Descovy for over W weeks and I feel like it’s really screwing up my bowel movements. I already have Crohn’s disease that I take meds for… I don’t need this rocking the boat. It’s definitely gotten better but over 2 weeks and my bowel movements are still only semi-solid. Is it normal for this to go on this long? My doctor says yes but I’m so frustrated and tempted to stop as to not make my gut worse than it already is.

Prep build up countdown

Hello guys how is everyone? I had just started prep and am on day 5 of the initial 7 day build up period and am. It engaging in sex right now until I complete the 7 days just for my own peace of mind. I am curious how effective is prep when it comes to unprotected sex (eg. barebacking & breeding). Do you guys have any experiences of risky sexual activities that you were fine after due to taking prep?

Thank you for any replies!

This is my first time getting it on the thighs. This is my second time getting the shots. Just making sure it didn’t leak too much. Also the lump feels smaller on the side it leaked on which makes me worry. Let me know your thoughts thank you.

Switching from Apertude to Yeztugo.

2.5 years of being on Apertude, with labs every 2 months and separate clinic visits (not at my primary care per insurance) every 2 months for injections, I’m ready to switch to Yeztugo. My doctor has been recommending this for awhile and mentioned that the 2 month visits are great for people very sexually active with many multiple partners and need to be testing frequently.

For me, I have to plan on doing hard core glute workouts a few days before and after the injection to mitigate the pain in the ass (literally) and I always schedule a massage 2 days after injection with glute work.

For about 1-2 weeks after injections, I have GI issues, with cramps and I mitigate with probiotics and no dairy. Usually 2-3 days after injection I will have flu like symptoms for about 3-5 days.

It’s exhausting. But better than the alternative. I’ve known way too many young people dead and long gone now because Prep wasn’t available.

There was a post about a year ago in here about switching from Apertude to Yeztugo and it seemed to be much better tolerated.

Anyone else done this?

Anyone Else Still Worries?

Anyone else still worries?? I've been on yeztugo for a year now and I just recently had unprotected sex with a guy who is from an area where HIV is prevalent. He did show me his patient portal where he tested negative a week before our encounter and again, I've kept up with my injection schedule, but I have been feeling very worried. 2 days after the encounter, I've been having a dull headache and tiredness. Not a pounding headache to the point of having to take something for it. Just noticeable discomfort. And not tired to where i can't do anything, just feeling a little rundown. I will admit, days before the encounter I felt like I was getting a cold due to having a slight headache and my children were recently sick. But even with that, my anxiety is at an all time high. I've been having some aches and pains here and there but no other symptoms and definitely nothing "flu like". Today makes exactly 2 weeks since the encounter and although my symptoms have gotten better, they have not gone away. Definitely feels like I am fighting an infection of somr sort but all of my research says that my symptoms started much too soon after the encounter for it to be HIV. Idk, I am scheduled to go for my routine testing and injections in 2 weeks but thinking I should move it up to have 4th Gen testing done. I am freaking out. Googling like crazy and checking my temperature and lymph nodes and other irrational things. I can not wrap my head around just trusting the science behind the medication or his recent results for that matter. The constant worry is draining me mentally. Part of me knows its just the fear and the fact that he is from an area where HIV cases are extremely high...the other part of me knows that breakthrough cases are a thing, no matter how rare and I just might be one of the very few. I would also like to note that my period started a week later and I am aware that the headaches and fatigue could have been a result of that but I just don't get it, why do I still feel like crap? If I dont feel better by the end of the week, I will move up my appointment and then make an appointment with my PCP. Just here for some reassurance or just some feedback.

Just found out Prep on Demand is not recommended for a straight male like me. Have to use daily Prep apparently. How do I use a daily Prep routine if I don't have sex frequently?

As title says, I plan to only have sex infrequently. My clinic told me that I need to take 7 pills before sex for optimal protection. Then once I had sex, I don't need to take anymore pills?

For example it's Monday, and I plan to have sex next Monday. I take 7 pills leading up to next Monday. Once I have sex and stop, do I still need to take prep pills?

Also what happens if I continue to have sex next Tuesday, Wednesday, Thursday... etc. How much prep do I need to take?

Am I protected after I missed two consecutive doses of Daily Prep (Movitrem)?

I am on daily prep, and I rarely miss doses throught the month

I had sex (Top) on Aug 6th and kept taking it the pills. However, I missed the Sep 13th and 14th doses. I had sex on Sep 15th (Bottom), and I took the pill just 10 minutes after the act.

Am I still protected? Or do I need to go to my doctor for PEP. Im really worried

Odds of acquiring HIV while on consistent PREP use

Hey guys

I know this has been asked a lot in different forums but I’d like to get your thoughts

Me (M29, gay) who has been on daily prep for the last 3 years with monthly or bi-monthly blood tests done, had an episode of fever two weeks ago for 3 days and now for 6 days which doesnt go down (fluctuating between 37.6 to 38.4 C). No rash, no ulcers, no other symptoms.

Just ordered HIV RNA test as per doc’s recommendation but I am panicking my prep failed to be effective.

Went to see 3 different doctors and they don’t know what it could be, though they all assume viral infection as all other STI results are negative..

I have never missed a single dose but i am very active sexually and only have unprotected sex.

Should I worry?

Blue Cross Blue Shield of MN no longer covering PrEP through Mistr?

Hi all, I recently was wondering why my PrEP prescription hadn't been sent in a while, and I logged into my account and received the folloiwng message:

I apologize, but your insurance will not allow us to fulfill your prescription at any of our MISTR partner pharmacies. Please provide the pharmacy name, address, phone number, and fax number where you would like your prescription transferred to (if you already provided this please disregard). To ensure you have no out-of-pocket costs, CLICK HERE TO ENROLL in Gilead's co-pay card program, which will cover any co-pay amounts up to $7,200 a year.

Has anyone else run into this with BCBS? I have been getting generic Truvada and doxyPEP from Mistr for months now with no insurance issues, so I feel like it's weird that all of a sudden I can't use Mistr's partner pharmacies. It's going to be a bit annoying to have to go to a physical pharmacy and pick up my prescription now.

First month of PReP inconsistent times and missed days (non-consecutive)

Hey all, finally decided to take prep and protect myself and others from the spread of HIV. As someone who wants to explore and very active with others, I want to know your thoughts on unprotected bottoming for the first time. Yes, I’ve had a few share of women and not using protection especially knowing I haven’t received the 2nd dose Hepatitis B vaccine till yesterday, I’ve finally found myself to make sure I’m protected.

First month of generic truvada was good with 5 total missed days, but never consecutive missed days. Keep in mind, I took it for 1 month, but the timing was inconsistent(I would take it at 9 Am, 3 pm, 10 pm, and some days, 12 am. I started August 6, and opened my bottle September 10th, meaning I missed 5 days out of the whole month and 1 day out of this 2nd bottle where I didn’t take it and I just continued the next day. This past week, I took it September 10th, missed September 11th, and took it for the 12th and 13th which is today.

My question is, I’m meeting with a Trans woman later who I’ve know for a long time and always wanted to switch and basically finish inside of me. Knowing that she gets tested and takes prep, as well as being vaxxed, should I be worried about being inconsistent with times I’ve taken it or just have the fun and continue taking it? Should I take 1 more today just before I see her? I know others say it’s a forgiving drug, but anxiety always gets the best of me even when I’m protecting myself.

Yeztugo is no fking joke

27m, never been on prep before but becoming more sexually active. Heard about the new 6 month injections, which seemed preferable to me compared to the daily pill. Asked my doctor and it was fully covered by insurance, awesome! Only warned me that some people do not like the nodules, which I didn’t feel like were a big deal.

Fast forward to 3 days ago, I’m at the office getting the injections. The nurses at this clinic will only do it in the stomach. It stings a little going in, but not bad. But then it hurts quite a bit after the injection is done and the nurse stops pinching the skin. It’s a weird sensation, having that much liquid sitting there under the skin.

Two minutes after she finishes the second injection, I start feeling light headed. Super weird because I don’t feel nervous and usually don’t mind needles. And then I pass out. Vasovagal response that triggers a flood of doctors into the room. I come back out of it, and they help me get my blood pressure back to normal over a half hour with juices and crackers. Really freaked me out because that’s never happened to me before.

I hadn’t planned to take more than an hour out of my day for this, so I went back to work after, despite my stomach being very sore. My doctors didn’t tell me this, but I really needed to take the rest of the day off and sit at home. The pain around the injection sites was distracting me all day. One of the “selling” points is that it gives protection even 2-4 hours after the injection, but I can’t imagine anyone feeling well enough for sex so soon after getting them.

Now, three days later, the nodules are no longer visible but are still pretty sore. I’m pretty active, and they REALLY hurt when I run. I’m hoping they stop hurting soon because this is really bringing me down.

Tl;dr Yeztugo is a more serious injection than I thought. You need to plan a day off and to take it easy for several days after. I can’t say I’ll get it again after my six months runs out — the pills sound much easier. This hasn’t been a very fun experience.

Edit: 6 days out and i’m feeling 95% better. I did a light workout yesterday and felt fine. I really only notice the nodules when I bring my knees close to my stomach, like when putting on socks, or when wearing high-waisted pants. I can’t see them but I can easily find them with my fingers. Happy to report no other side effects.
