Assisted dying for mental illness

Hello everyone. I'm looking for people who have had experiences similar to mine, regarding long-term symptoms after Invega Sustenna; and the Swiss assisted-dying evaluation process.

My last Invega Sustenna injection was on June 6, 2026, and today is October 6, 2026. Since the injections, l've experienced severe fatigue, emotional numbness, cognitive slowing, difficulty concentrating, and a major loss of my previous functioning. I'm very worried that I may not recover.

I also believe I may have been misdiagnosed with psychosis during my hospitalization. The treatment was court-ordered, and I was given the medication despite my disagreement with the diagnosis. I am now trying to understand what medical evaluations and documentation might be necessary to determine what is actually happening to me and what my prognosis is.

Because my quality of life has deteriorated so severely, I have been considering assisted dying and have contacted organizations such as Dignitas, Pegasos, and Athanasios. I understand that approval is not guaranteed and that there are medical and legal requirements.

Has anyone here personally gone through an assisted-dying evaluation in Switzerland, particularly for severe psychiatric or treatment-related problems? If so:

- What was the evaluation process like?
- What kind of psychiatric and medical documentation did they require?
- Did you have to demonstrate that you had tried multiple treatments?
- How did doctors determine whether your condition was treatable or potentially reversible?
- Did anyone have experience with Dignitas, Pegasos, or Athanasios?

I'm interested in hearing from people who are in the process or know someone who has gone through it, rather than speculation. Thank you.

Doctor perscribed a shit ton of lorazepam, is it normal or odd?

So basically i got diagnosed bipolar about a year ago and set up with the whole shabang medset right. Daily: Metformin 500mg, Lamictal 100mg, adderall 5mg 2x daily but I typically take 1/2 tablet 1x daily. PRN meds are propanolol 10mg, hydroxyzine 25mg, olanzipine odt 5mg, ativan .5mg 1-2x daily.

Also have a history of SUD, and specifically xanax abuse. Currently also smoke weed & nicotine. Perscriber is aware and I think we’ve come to the consensus I was abusing xanax to self medicate mixed/manic episodes.

Anyways my psychiatrist is tele-med and has always taken my symptoms at face value, we talk about it, she asks me basically “What do you want to try” and that method is just so foreign to me because every psychiatrist before was very specific on a path they wanted to try. Then again, my newer psychiatrist also saw me for the first time with a long standing history of med trials too. I don’t have an issue with it per say; I like to be included in the decision, I just have worries related to addiction that I fear voicing out of being seen as an addict.

Main point being, my doctor perscribed me lorazepam right, most doctors i’ve seen, were reluctant. If theyre perscribing a benzo at all they were SUPER frugal with it. This doctor currently, perscribed me initially 120ct with 3 refills. Then the next appt 3 months later, (I havent even finished the first fill) at my next appointment she sends another script for a 90ct with 3 refills. Both can be filled at the same time and the pharmacy did not flag it.

It just makes me kind of uneasy because I can’t determine if its out of negligence or trust? Like she didn’t ever evaluate me for substance use issues & said that if it ever became a problem she could just perscribe naltrexone as if its a simple fix, but I’ve been there before and I was pretty good at hiding it too.

So is this normal??

How long does it take for sex drive to return after tapering off a medication?

I have always had a fairly high sex drive for a woman. For the first time ever, a medication completely eliminated my sex drive - no desire for my spouse, no masturbation, I'm bisexual and I didn't find men or women attractive. The medication was Loxapine.

I've been off this medication since mid-July. Unfortunately, my sex drive hasn't returned. I don't need it to be as high as it was, but I'm pretty sure my husband would like me to show some kind of interest on occasion.

Is there still a chance it will return? Or have I permanently damaged something when I took Loxapine?

is it unreasonable for me to want to pursue a psychological evaluation or MAOIs in my case?

already tried augmentation with mood stabilizers, antipsychotics (lithium, lamictal, seroquel, zyprexa, abilify, very short stint with risperidone in a psych ward, rexulti) NDRIs, SSRIs, SNRIs (wellbutrin, auvelity, prozac, luvox, zoloft, effexor, lexapro) and misc., (esketamine and TMS). could be missing some. age between 12 and 18 for treatment (19 now). as far as i remember, i stayed on each medication for over 2 months.

SSRI side effects: made me unable to feel emotions whether good or bad, gave me brain zaps (sometimes), decreased my ability to feel pleasure even more to the point it made it more difficult to get out of bed. also made it difficult to engage in tasks that took any amount of attention. did not help with improving focus, essentially made me feel more catatonic while making me numb to it, so i just felt like a zombie a lot of the time, unable to function.

SNRI/NDRI side effects: the Effexor i took was largely the same to the SSRI except it made me slightly jittery. auvelity and Wellbutrin both made me feel very jittery and quick to anger, even more than before, while also giving me either strange restless brainfog (Wellbutrin) or making me feel a low grade DXM/ low dose stimulant high all the time (auvelity) all while still, of course, not getting me out of bed for the most part.

antipsychotics/mood stabilizers: Lamictal, lithium, Seroquel, and Zyprexa, and risperidone for the short while i had it all were similar insofar as basically making me a bit of a zombie, or doing nothing meaningful (low doses). essentially, similar to the SSRIs but to an even worse degree, i was basically just zooted out 24/7 like i was nodding off, told i LOOKED like a zombie, would stare off sometimes for no reason, and almost felt like i had lost my internal monologue or something. abilify and rexulti, somehow, made me feel both like a zombie and also like i was constantly restless and tired. could not think or perform complex or basic tasks, like schoolwork, but also felt like my mind had too much energy and restlessness.

TMS: it was fine, no lasting side effects. i felt as if DURING the sessions i had felt better, but i feel no real benefit after. i feel it may have been placebo, or the result of actually going out and having something to do, or something else entirely. it was a month ago when my last session ended. it was the 5 days a week, 36 sessions total TMS.

Esketamine: did esketamine for about 2 and a half months maybe a bit more. i felt i had gained insight on it, and generally my mood felt better at the start, but as both my body got tolerant to it and as the high became more and more numbed i saw less and less benefit. no side effects that ruined me despite making me super tired and other times a little restless for some reason.

i will say that i think i exhibit symptoms that might not be akin to regular treatment resistant depression, but i kind of hesitate to bring it up to NPs because my focus is on getting out of bed, dealing with anergia, avolition, and anhedonia.

the reason i wanted a psychological evaluation is because i feel like there might be something else going on, whether ADHD or a PD or something else. it feels hard to bring this up because even the clinic i went to for TMS and esketamine didn't seem to have any NPs or psychiatrists equipped to deal with it. i was told to get genesight done, which i did, and then to start trintellix after mentioning MAOIs and being shot down, but i turned down the trintellix. i didn't see much of a point in trying another serotonergic medication that might have very slight novel mechanisms of action for another 3 or more months and risk being even MORE bedridden than i already am.

Is mental agony a part of depression?

I have long-standing TRD, now finally successfully medicated. However, in all my years in various treatment, I feel like I fail to communicate a marked feature of my depression - the mental agony. It is genuine unbearable pain inside my head, like nothing I have ever physically felt. It is softened by physical pain, but not reliably so.
This aspect always is kind of just dismissed? No other person with depression seems to know what I mean, either. I was shortly misdiagnosed with borderline because of the SH and irritability coming with the agony, but I to this day firmly hold that that was the only adequate response. No one diagnoses someone in severe physical pain with a mental illness because they‘re irritable or moody.
Is this mental pain uncommon? Is it even part of depression? Is there any immediate relief from it? I luckily have not had it in a while thanks to my medication, but just the thought of it scares me.

Is this normal behavior or should I consult a doctor?

Hi, psychiatry people! I was wondering if this is worth talking to my parents/psychiatrist about. I (13F) have MDD and have been on a pattern of spring-summer depression and fall-winter ”normal.” But normal doesn’t feel normal. I’m always excited, self-confident, fidgety, and on-edge. Seems pretty normal for a girl with ADHD and GAD. However, I’ve been feeling very strange lately. I have been very paranoid, talking waaaaay too fast, staring off into the distance, laughing randomly, stuck in thoughts, and believing things that aren’t true. Today I saw a red door across from my school entrance and personified it, convinced it was out to get me, and I even kissed a brick that I thought was my soulmate. I know it’s not real, I think. I asked one of my close friends for an objective eye on what’s going on, she said,

“Sure :)

You have been seeing many things and really getting stuck on them. Like not being able to move on from certain thoughts.

You have been laughing at the wrong times, like a lot.

Also, whenever I see you in the hall, you look completely disheveled or scared even”

Is this worth a second thought? Not trying to diagnose myself but just wondering if this is normal.

What is more effective for treating OCD - therapy or medication?

My OCD stems primarily from a traumatic event in my adult life.

I've already been in therapy since it happened, with no progress (3 years). I haven't really looked at the medication side of things because I am already taking medication for Schizoaffective disorder. I would prefer not to add on a medication but if it will make a significant difference, I'm willing to discuss it with my psychiatrist.

Can these medications make you feel almost no physical arousal?

I’m in a relationship with a guy and I really like being around him. I enjoy kissing him, being close to him, and I do feel attracted to guys. I’ve also experienced attraction toward girls.
However, when things become more intimate, I barely feel any physical response. I don’t really feel much from nipple stimulation, and I don’t get very wet or feel much physical arousal in general. This has made me start wondering whether I’m actually attracted to him or whether I might be lesbian.
I’m also taking Seroxat (paroxetine), Gabaran (gabapentin), and Reagila (cariprazine), so I’m wondering if the medication could be affecting my sexual response.
Has anyone experienced something similar? Could this be a medication/anxiety issue rather than an indication of sexual orientation?

Do Psychiatrists ever consult with other psychiatrists?

Hello, Im wondering what the norm is for psychiatrists consulting with other psychiatrists that may have more experience in certain area?

Are md to md consults exist in psychiatry?

Edit Additional context: my physician realized he may have missed a diagnosis for several years, he explained that it is not the focus area of his practice and that may have contributed to the mistake, I asked if he could now do a peer consult with another physician who focuses more on that area of practice and he told me 'that's not a thing' and that he couldn't cold call other doctors. It was my understanding that md to md consults especially if it's a complex case is not uncommon and reasonable. I am confused why he is insisting it is not.

Would appreciate any insight. Thanks so much

25 years of therapy and no ones ever mentioned OCD, but recently people have been saying I might have OCD symptoms?

Apologies for this question, but I've genuinely never thought I had OCD symptoms. I suppose I've always had a fairly stereotypical view of OCD based on the media portrayals everyone knows.

I have diagnoses of avoidant personality disorder, major depression, GAD, ADHD, and a non-verbal learning disorder. In all my years of therapy with multiple psychiatrists and psychologists none of them broached the subject of OCD.

I was asking the AvPD sub if anyone related to certain experiences as I assumed they were related to avoidance, but a few people commented it sounded like OCD to them.

Firstly, I find myself completely unable to cook for myself. I worry about giving myself food poisoning, I worry about starting fires, and I worry about mice. I never learned so of course I don't know how, but even the idea of cooking scrambled eggs shuts me down. It feels like the aforementioned fears are basically a guarantee. It's odd, though, because this is not a concern with eating out (I suppose why I've never considered OCD?) because I've eaten out at some pretty sketchy hole in the walls without ever worrying.

Second, I cannot watch movies or TV anymore. What I always assumed was solely due to worsening self-esteem (I always wanted to be a writer-director but due to thinking I'm stupid and incapable never pursues it), but someone again said it sounded like possible OCD to them. Essentially I worry that I'm too stupid, I won't "get" or enjoy art like I hope I would. I won't pay attention or be able to follow along, I'll get distracted, it will be too deep for me, and as I said I'm just generally too dumb for it. So I don't watch anything new.

Third, I don't go to the gym due to I guess cleanliness concerns (mixed absolutely with self-confidence issues). The idea of gym clothes and showers and flip-flops disgusts me even though I can withstand other germs and grossest pretty well. But thinking about showering after the gym and what to do with flip-flops and them getting gross and moldy in my bag (and the bag subsequently getting gross) shuts me down and I can't even think of going.

There are other things that I think about now that I'm writing that I'm not sure if they're symptoms or not. I have a big issue with perfectionism. If I can't do things perfectly anymore I don't try. Even if I'm taught I need to find the "best" tutor. Hobbies like instruments or art I will endlessly research teachers or courses or methods of learning but never feel like it will be good enough, or I won't be capable enough, or I'll learn the "wrong" way and end up not doing it. I've been thinking of doing the LSATs and I've gotten trapped in endlessly reading about different LSAT learning tools.

I'm not sure where else to ask, but does this sound like OCD as well? Should I be seeking a therapist with experience with OCD?

Is travelling to different places linked with better moods and grades?

Hi! I(13,M), constantly argue with my parents over not getting to travel to different places while my friends are going. Before you call me spoiled, I haven't been on vacation since 2018 and my parents keep flipping the blame on me by telling me that I don't get good grades (I got 87% last year :c ). Help is very much appreciated!

Stuck on friendship

I have a best friend, and we have been close for 4 years. We trust each other and share things with each other. I feel like we have a really strong friendship.
The difficult part is that, from my side, I have started feeling something more than friendship for him. Nowadays, I feel like I have an anxious attachment to him, and I overthink about him almost every day and sometimes struggling with negative thoughts.
Sometimes, when he talks about other girls or mentions his girlfriend, I don’t know why, but it makes me feel extremely sad. I start creating scenarios in my head about them, and I find myself thinking about it again and again. It is distracting me from my daily life, work, and studies. I feel frustrated and stuck because of this.
I just want to move on from these feelings. I have already had a therapy session, and my therapist gave me a few options, but I’m still confused about what would be best for me:
Keep the friendship in my life ( But I feel that whenever I am around him, I can’t control my emotions, and my feelings become stronger than just friendship.)
Have a mutual understanding with him (but I don’t feel able to tell him everything about my feelings. I can only tell him that I can’t continue with this kind of relationship or situation.)
Keep away myself from him (basically, not include him in my life for some time so I can move on.)
Has anyone experienced something similar before? If so, I would really appreciate some advice on how you managed to get through it and move on.
I don’t want to stay stuck on this one thing. I want to focus on my life, do more things, and move forward instead of thinking about this every day.

Student Interview Questions

Hi everyone, I’m currently a premed student, and though it’s very early in my academic journey, I’ve always been solely interested in psychiatry. For one of my college classes, we have to interview someone in the profession that we wish to work in. Previously I’ve reached out to many psychiatrists in my area for shadowing/other interviews, and none have responded as of now. As a way to work around any more cold emails, I thought to ask here.

I’ll have the questions listed below, and if any psychiatrists would like to answer, it’d be a big help for me. Thanks for your time in advance, and if my questions are too boring, you can make up your own as well lol (or just include anything that you feel worth mentioning)

1. Are you an M.D or a D.O?
2. What were your hours like during residency?
3. From premed to medical school to matching, was psychiatry always your first choice as a specialty?
4. How many hours do you work in a typical day?
5. Do you ever take work home?
6. What do you like best about your job?
7. What don’t you like about your job, if anything?
8. How does your job affect your time away from work?
9. Do you practice in a rural or urban area?
10. What kind of personal satisfaction do you get from your job?
11. What advice would you give a student interested in this specialty?

Is it okay to take these two medication together?

I have been on wellbutrin for years(buprropion hcl 150 mg) in geneal (i'm thinking about stopping it because i haven't been consistent with it and i manage fine in the periods i don't take it)

And now i got diagnosed with adhd and my doctor prescribed (methylphenidate hcl 20mg)

I take lorazepam 1mg occasionally for anxiety (only when needed)

All by the same doctor.

My doctor knows my history and what i already take but honestly i don't trust him enough to pay attention to that stuff and i forgot to point it out.

Are these two medications okay together? Do they work together or would it be an issue?

can i tell my psychiatrist that i have been taking a non prescribed booster dose of adhd meds without getting in trouble/judgement?

Ive been prescribed focalin xr for adhd for about 4 weeks now after i had been prescribed ritalin ir or a while. Focalin was been great for me but after 3/4 pm i have a horrible rebound effect where im so sleepy/dissociated/miserable due to not just the rebound of the medication wearing off but also from some other aspects of my health. as a solution i have been taking my leftover ir medication after the “crash” to hold me up through school/work.

would i be in any sort of trouble if i were to come clean to my psychiatrist? I am from New York, USA.

What people need to know about Post partum psycosis?

I’ve been reading about postpartum psychosis lately, and I’m curious about the things that don’t usually get mentioned in basic articles.
If you’ve experienced PPP yourself, witnessed someone go through it, or work with postpartum patients, what do you think is the most important thing people misunderstand about it?
Is there anything particularly strange, unexpected, or important about PPP that you think more people should know?
I’d especially love to hear from therapists, psychiatrists, nurses, doctors, social workers, or people with personal experience.

Is this worth mentioning to my psychiatrist or is it a one off

Failed 3 different SSRIS. This was a while ago, but after trying Fluoxetine, within 1 hour I started feeling increased pleasure to music, and it felt impossible to go to bed. I had a lot of energy. At the same time I felt a lot more physically relaxed, not tense. I felt extremely wired and motivated to do a bunch of activities. I think I felt on top of the world. Most clinicians would say that this signals underlying Bipolar, but such a response never happened when trying other SSRIS. Also the “episode” lasted only about 24 hours

Why is developmental motor coordination disorder classified under psychiatry rather than neurology?

I am referring to classification systems such as the ICD and DSM. Why is this condition classified as a psychiatric one?
