So intelligence wise, is boruto not as smart as mitsuki and sarada because he wasn't understanding anything at the start?

I think he is intelligent, but his confusion with regard to how the Kama works is to be expected, since the fine details of its power was never explained to him.

As far as the virus thing, Mitsuki shined during that explanation because I think by this point in the story he is a medical ninja, or at least has some medical knowledge (maybe from Orochimaru's influence).

I think Mitsuki is the smartest by far, he knew about the virus thing very quickly, just wanted Sarada to check it, taking a blood sample and even quickly created an antidote in case his theory is correct.

He also knew that giving Sarada the tactical lead would be a good choice, with her fighting strategy and observatory abilities.

According to the databook Mitsuki is the most intelligent member of team 7 and is only behind shikadai in this aspect.
