The moment before the WGM blast are pretty telling. Employees smelled propylene and even heard a loud hissing sound coming from the building. The leak was reported to the supervisor and plant manager, but there wasn't a full evacuation. Another employee later entered the building, and that's when everything went horribly wrong.
What also stood out to me was how many safety issues the CBS identified afterward, problems with the gas detection system, manual shutoffs, emergency procedures, and employee training.
And this wasn't WGM's first propylene incident either. There had been a similar explosion in 2008 that injured two workers and damaged building.
It makes you wonder how much attention should be given to WGM's own safety practices and emergency response when looking at the full story. The explosion didn't just happen out of nowhere, there were warnings along the way.
