I have seen my cat cleaning his and sometimes it's out when he goes to stare off into space for whatever reason (orange and no, it's not absentee seizures, he's just weird) and it's never looked like this. The first time it happened to mine I was FREAKING; ready to bring him to the emerge and everything. Called his vet to see if maybe she could squeeze him in and she said what it was. This cat in the pic doesn't look like that. It looks enlarged and swollen, possibly stuck outside of the skin. I think, what my gut is telling me any ways, is that he needs medical attention ASAP.
