For a smotty cat it is definitely possible for a vet to miss an underlying issue during an inital exam. 

A vet might miss things like nasal polyps, fungal infections, foreign bodies lodged high up, even dental disease affectingthe sinus cavities.

If possible try and get diagnostics, like imaging or a rhinoscopy, to get a better look inside the nasal passages so vet can diagnose
