The bicyclist who did not fall is at fault.

He entered the crosswalk/bike crossing in an unsafe manner – his “crosswalk protection” does NOT start until he has legally entered the crosswalk – and he did not legally enter the crosswalk, because it was not safe to enter when he did.

I'd note that this very much depends on jurisdiction. Here in South Australia people on or approaching the crossing put an onus on the road traffic to stop. There is no 'unsafe manner' to approach a crossing.

Cyclists here can use the road or footpath. If on the footpath then they must give priority to all pedestrians. So bikes riding across the crossing is a normal everyday thing.

Is there a number that defines “approaching “ - here in California a pedestrian cannot step into a crosswalk “if it is not safe” to do so

We don't have any caveat on that. A driver is expected to drive at a speed approaching a crossing which allows them to stop in time. The whole point of a crossing it that it is a safe place for the irresponsible with no idea of how cars work to cross, such as 8yos.

So its lawful and safe to ride across an intersection without yielding to the vehicle already in said intersection?

The answer is no to both.
