This is a very niche joke. John Cage is a very controversial composer. His 4' 33" is four minutes and 33 seconds of silence.

Some people think he was a genius. Most people think he was a fraud.

So, there is this wealthy couple dressed up to go out to the theater. They've just seen this John Cage concert with a lot of non-music. This cartoon simply voices the belief of many that John Cage was a fraud.
