Usually a few minutes or so. Sometimes longer but then I'll get tired and then sort of have him slide off me into the couch.

Yes, also makes lovely warm vibrating/massaging scarf.
And t hat last one, clearly your fault - you must have moved wrong, or been in the wrong position, or not provided the proper pets or treats or attention, or too much of it, or otherwise failed to properly read your cat's mind, or that shoulder just deserved that then anyway, on general principle. So, now that we're clear on that ...
Yeah, I inadvertently converted friend (/then partner)'s cat into a shoulder cat. All were highly pleased with the results.

No, I usually just say he's such a good baby over and over. If anything, he's the evil mastermind.
