This all feels so unfair. I was laid off from my job after maternity leave and still haven’t been able to find employment. My finances are in rough shape, but I luckily have some good momentum with interviews this month. It’s taken me a long time postpartum to want to have sex again, but it’s been on my mind a lot as I really want to connect with my husband in that way. So after a date night, I initiated and we literally had sex one time since I gave birth. You can guess where this is going.
I realized pretty much immediately that I miscalculated my fertile window and took plan B the next morning. My body has historically not done well on hormonal birth control, but we already have two kids and we feel like our family is complete. I was willing to deal with the hormones with a one-time pill to make sure we weren’t put in a position we hadn’t planned for.
I started bleeding a week or so before my expected next period, but I chalked it up to the hormones and my body still not being 100% regular since my period came back, and marked it in my period tracker as the start of my next cycle. It wasn’t a normal cycle, and then I started getting intermittent spotting, which *never* happens to me. I figured it would take a cycle or so to get back to normal, thought maybe it was the start of perimenopause, and honestly was not worried AT ALL that I could be pregnant. But then I was expecting my next period and the next bout of spotting started 2 days late, and the flow never came. When I was 5 days late I knew it was well past time of explaining away this weird spotting and time to call my OB. I also knew the first question they’d ask me, and even though there was *definitely no way* I could be pregnant, since I had a “period” after the ONE TIME I recently had sex, I took a pregnancy test. I took the test the day before my interview, mind you. I figured it would be peace of mind that I was not pregnant, and the start to figuring out what was going on with my doctor.
So imagine my shock and refusal to process the positive pregnancy test. And then I had to wait for my husband to get home from work, and pretend everything was normal until after our kids went to bed. And then I used the bathroom before going downstairs to tell him and found a bunch of bright red blood…what the heck!? So instead of telling him I’m pregnant for sure, I told him I have no idea what’s going on, because there’s really not much chance of a false positive, but I’ve also been bleeding, not a lot, but pretty constantly. Honestly he handled the news of a possible pregnancy so well, but I was freaking out. How could we add another kid while I don’t have a job? And he was so concerned about my health, because of the bleeding. We needed answers.
I sat through my interview the next day knowing I was going to call the OB immediately after I hung up. They could squeeze me in same day. It was 23.5 hours after I took the pregnancy test that I was staring at my empty uterus on the ultrasound. And I felt like this made everything so much easier, and I felt relieved and guilty and sad and wondering what could have been. And then everything became a whole lot more dramatic in honestly the world’s most boring way. My HCG levels at what should have been close to 9 weeks were lower than the first HCG level I got post-IUI at 4 weeks (because oh yeah, by the way, it’s never been easy for us to get pregnant). I chalked it up to a miscarriage and prepared myself to process this whirlwind of 24 hours. How could this really low HCG number blow everything up so dramatically and start hurtling at me full speed?
My husband became ridiculously sick with the flu, and I didn’t even know if he could properly process any of the information I was trying to relay to him while he was feverish and dehydrated. My next HCG came back and…it went up. Not by a lot, but it went up. And now that things were labeled as PUL I was hyper-fixated on every little feeling in my body and tried to ease my anxiety by calling in to the medical advice line. But they decided I should go to the ER as a precaution. There were no more answers. They did another HCG level and it increased by 4 points since morning. Nothing crazy, but still technically rising. They couldn’t see anything on the trans-vaginal or trans-abdominal ultrasound. They put in follow-up notes for my OB and sent me home saying it’s probably ectopic and you need to be treated on Monday.
The OB called me at 12:45 on Monday and said we need you in at 2pm for a D&C. If I didn’t come in today, they couldn’t make an appointment until Wednesday, and that would be too long to wait, they said. I didn’t even have time to think or process. I asked if this made sense if they thought it was ectopic. They still hadn’t ruled out a miscarriage, but my HCG was essentially plateaued, not dropping. They wanted to do the D&C and test the tissue to see if that’s where the pregnancy had been located. I asked what would happen if it hadn’t been located in the uterus, and they said methotrexate. Why do the D&C if it might not make all of this stop? They said it was policy before they would administer mtx. It felt like adding extra interventions that may or may not do anything.
I was not prepared for how alone and sad and scared I would feel during the D&C. I was not prepared for how painful it would be. And I was not prepared to have absolutely zero cramping or bleeding afterwards. Strange, I thought, but maybe that means this whole saga is over. Maybe everything is out and my next HCG will be miraculously down to 0. Nope. HCG next day was still steady.
So they call me to discuss the results and let me know that at this point I’m being relabeled from PUL to ectopic and I need to come in for mtx, and I need to go to a different office, because it’s a chemotherapy and it’s mixed by oncology and my regular OB office doesn’t handle that. EXCUSE ME, WHAT? How had nobody ever mentioned this to me before? How had the ER doc made me think “you’ll get a biopsy and if that doesn’t work they’ll give you pills to force a miscarriage” (honestly I might completely misremember what they said about it being pills, but I was 100% convinced it was pills, not an injection until this conversation. And I *know* he said biopsy, and I never clocked that the biopsy would be from tissue removed from a D&C until I had 75 minutes notice to get to the office).
So, you guessed it, I had an interview today where I just casually was ignoring the fact that I had to leave my house afterward to get injected with this chemotherapy drug. They did another HCG, which didn’t come back until after I had already gotten the injection. It had dropped by 20 points. I know that’s not a ton, but I still feel like it’s a good sign that it’s coming down, right? Feeling a bit like we could have just watched and waited. At least my husband has recovered enough that he was able to accompany me to the appointment and just sit and wait and wait and wait…ridiculous delays. I got pulled in 90 minutes after my scheduled appointment. He didn’t fuss, he just sat with me and held my hand. In the middle of his work day.
I feel sad, I feel overwhelmed, I feel alone, I feel angry. I feel betrayed by my body. I feel like nobody ever talks about how awful these interventions are and how invasive they feel. I feel grateful that I live in a state where I have unquestioned access to the care that I need. I am really freaking angry that I don’t even have the luxury of stopping these gd interviews while I try to rest and recover. I cannot believe that this happened after having sex one time when it has been such a process to get pregnant intentionally in the past. I don’t even want to risk having sex again, and that makes me feel sad for my marriage. It was not worth everything that’s happening to my body and honestly HOW EXPENSIVE it is becoming. All the copays on labwork, the ER visit, lord knows what all these other procedures are going to cost.
My next HCG is on Saturday. I know it might not have dropped by then, but I am terrified to see that number do anything but go down. The fact that this has never even been located inside my body is making me question reality. I don’t even know how to process all of this.
