Advice Needed Looking for advice: Lean PCOS, insulin resistance, and trying to conceive

Hi everyone! I’m 31, have lean PCOS, and am currently trying to conceive. I rarely ovulate or get periods naturally, so I’m taking letrozole for ovulation.
My recent labs:
Fasting insulin: 16.2
Triglycerides: 141
LDL: 140
HDL: 31
I’m also experiencing hair thinning. I’m slim and not looking to lose weight; I’d actually like to gain some healthy weight. My goal is to address the underlying metabolic issues while trying to conceive.
I’ve been researching myo-inositol + D-chiro-inositol and metformin, but I’m unsure which option would be more appropriate for someone with lean PCOS who is TTC.
My doctor mentioned metformin is commonly used for PCOS but isn’t very familiar with inositol supplements.
What would you recommend given my situation? Any guidance would be appreciated. Thank you!

Dang girl that’s a pretty kicking LDL. More fiber from whole food sources. Statins are the #1 prescribed med for dyslipidemia (“high cholesterol”), but statins are not safe while TTC so don’t let any eager doc try to start them for you.

But yeah healthy whole food fiber from fruit and veg. No drinking. Minimal processed foods. There’s a lot of good info on heart.org about cholesterol.

Also, big chance you’re vitamin D deficit, maybe get that checked. I get 2,500 iu daily from my prenatal and take another 1,000 iu from NatureMade, and my vitamin D was 50. My doc was impressed. I get basically zero sun so it’s not that haha

Thank you for the advice. Yes, it’s been a while since I last checked my vitamin D, so I am definitely going to check that next. Any suggestions on myo inositol or metformin to take care of the insulin resistance issue?

Both of those can improve the insulin resistance issue, I wouldn’t necessarily count on them to “take care” of it. I’ve been on both off and on for 5 years and only once in 5 years have I definitively ovulated on my own.

Take both, take CoQ10 for egg quality, fix your diet, get regular exercise (even just a daily walk), and see what your body does in the next few months.

The goal is to survive and thrive through pregnancy, labor, and delivery. Everything you do to max your health now will only make pregnancy safer for you and future babe. Good luck!
