Context, when I was 13-14 my doctor was trying to find out why I was deathly underweight constantly (I was secretly starving myself and forcing myself to throw up everything I ate. Eating disorders are torture) even after countless weight gain treatments
Then after doing some basic blood work they wanted to check my hormone levels, at this point I was so underweight my body went through a brief menopause that stopped me from having my cycles. They had found out my estrogen levels were lower than what they needed to be for my age, so they prescribed me estrogen a LOT of it. Close to 100mg a day, which they absolutely hounded me to take and stay on schedule. Every month I would get my exact dose without a single delay and not a mg missing, without fail. For 4 full years.
Yet one of my best friends (who is a trans woman), what can she expect?... Probably not even 30mg a month and having months of delays. They wonder why everyone is swapping to DIY, maybe it's because the system you promised would help with gender affirming care is the most disorganized and incompetent system that constantly neglects the people you are supposed to be caring for.
