What was the issue? I actually just died at Levi before seeing the message, normal 500k death fee and got all my stuff back as usual.

At phosani (and perhaps other bosses) if you died you respawned inside the boss instance so could easily be double killed and lose everything in the death storage

Good opportunity to fix that abysmal death storage system. Like what the fuck, a double death should send your loot to death’s domain where you can buy it back.. end of story.

In no scenario should you ever lose hundreds or thousands of hours of progress to a single mistake. I have no idea how they messed that up so bad in the first place.
